If you would rather order an application over the phone, please call the Admissions Department at 702-382-2200. Please have the following information ready.

1. Complete Legal Last, First and Middle Names
2. Address
3. Home Number
4. Work Number
5. Credit Card Type, Number and Expiration Date of Card.
